Change of Address Form for Children Currently Enrolled in Imagination Library through United Way of Southwest Minnesota

If your child already participates in the Imagination Library program through the United Way of Southwest Minnesota, and you are moving in the next 6-8 weeks or have recently moved, please use this form to file a change of address for your child. Please note that a separate form must be completed for each child you have registered.

Address changes take 8-10 weeks to take effect. If you are moving in the next 4-8 weeks, please feel free to let us know of the change that will occur and include the effective date of the move.

If we receive any books from the post office that are returned as non-deliverable for your child(ren), we will send you an email or try to call you so that you can pick up the book(s) at our office.
